Application Process
Jerome K. Lewis Scholarship Application Process
The awardee(s) will receive a one-time scholarship of $500 for one school year.
All applicants must follow these procedures:

Complete the Application: Fill out the application form and upload the required documents.

Essay Submission: Attach a 150–200-word essay detailing your community service contributions over the past four years. The essay should include a title page and be typed in 12-point Times New Roman font, double-spaced.

Photo Submission: Provide a recent photo to include with your application.

High School Transcript: Include a copy of your high school transcript.

Letter of Recommendation: Submit one letter of recommendation as a character reference.

Application Completeness: INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED!

The completed application package must be submitted and received on or before the designated date. No applications will be accepted after this date.
